Preparing you for life outside the hospital is our focus of The Center for Rehabilitation at Westlake Hospital. During your stay in our inpatient rehabilitation, you’ll work with a designated rehabilitation care team to design an individualized inpatient treatment plan based on your goals and individual needs. Our caring team will partner with you and your primary physician and focus on your return to independence.
We help improve your ability to provide self-care and to teach you and your family how to cope and adapt to changes resulting from impairments in mobility, safety, communication, cognition and/or swallowing. Your care is constantly monitored to achieve optimal outcomes and promote the highest degree of satisfaction.
Program at a Glance
- You receive 3 hours of therapy at least 5 days a week.
- The team provides extensive patient, family and/or caregiver training.

Interdisciplinary Team Approach
Our interdisciplinary care team is led by 24-hour coverage of a physiatrist (a physician specializing in physical medicine and rehabilitation). You receive coordinated services, which may include:
- Physical Therapy
- Speech Therapy
- Occupational Therapy
- Social Work/Case Management services
- Spiritual Services
We encourage you and your family to participate in our weekly team conferences and to take an active role in the rehabilitation program. Through active participation in your treatment plan, you are challenged to demonstrate personal responsibility and gain greater independence.
In anticipation of a patient’s return home, our rehabilitation team is involved in the discharge planning process, home assessment and homecare. The recovery process continues beyond discharge, as many patients return to Westlake Hospital for Outpatient Rehabilitation.
