Your Patient Rights

When it comes to your patient rights and responsibilities, Westlake Hospital wants you to be informed. Our policy is to inform all patients of their patient rights and we have an obligation to respect such rights.

Patients at Westlake Hospital have the right to:

  • Receive health care, regardless of race, creed, age, sex, sexual preference, handicap, national origin, or financial status, religious beliefs, and cultural preferences.
  • Be treated with dignity and respect.
  • Receive information (along with family when appropriate) about diagnosis and treatment plans, in language or manner easily understood and with an explanation of any alternatives, including risks, benefits and burdens of each alternative.
  • Participate and be involved in the development and implementation of his or her plan of care and treatment.
  • Request or refuse medical treatment to the extent allowed by the law unless request is deemed medically unnecessary or inappropriate.
  • Give or withhold informed consents to produce or use recordings, films or other images of you for purposes other than your care.
  • Choose a recommended medical treatment option appropriate to choice; particular age; special physical, spiritual, emotional, cultural and intellectual needs; and other living circumstances.
  • Expect that the same standards of quality are adhered to wherever a test or procedure is performed, whether inpatient or outpatient.
  • Receive an explanation and alternatives if a transfer to another facility becomes necessary, including information concerning access to protective services.
  • Be free from restraints of any form that are not medically necessary or not used to prevent harm to self or others, or are used as a means of coercion, discipline, convenience or retaliation by staff.
  • Have your pain assessed and help develop a plan to manage the pain you may have.
  • Participate voluntarily in or say no to research and clinical trials used in your care, without jeopardizing your access to care.
  • Know the names and professional responsibilities of their health care team.
  • Have personal privacy respected and confidentiality of patient care information maintained to the extent provided by law.
  • Access your medical records, request an amendment to, or receive an accounting of disclosures regarding your medical, and for a reasonable fee, receive a copy of your record.
  • Be heard, especially in the event of a conflict/complaint when the quality of the care or services received does not meet patient’s expectations.
  • Receive visitors that you designate, mail and telephone calls so long as they do not interfere with treatment. You may withdraw or deny consent for visitation at any time.
  • Receive information (to be provided to the family when appropriate) through effective communication about the outcomes of care, including unanticipated outcomes in which the outcome of care differs significantly from the anticipated outcome.
  • Receive care in a safe setting, free from neglect, abuse or harassment.
  • Be informed about continuing health care requirements — such as return visits and required medications — following hospital treatment.
  • Obtain a full explanation of the hospital and professional bills for care and services, and options for financial help if needed.
  • Formulate advance directives (with the patient having the right to amend or revoke his/her advance directive at any time as provided in the directive) and have hospital staff and practitioners who provide care in the hospital comply with those directives, with access to care, treatment and services not being dependent upon whether you have such a directive.
  • Have a family member or representative notified of a patient’s choice of advance directive and have family and/or the patient’s primary care physician notified promptly upon a patient’s admission to the hospital, if necessary.

Patient Responsibilities

Patients have the responsibility to:

  • Notify a family member or representative of advance directive so that we can know what kind of care you wish to have should you become unable to tell us. Notify physician promptly upon admission to the hospital. 
  • Provide accurate and complete information about present condition and past medical history to physician. Notify physician or nurse of changes in health.
  • Follow the instructions of physicians and nurses, comply with treatment plans and ask questions to clarify understanding or when in doubt.
  • Express concerns and/or disagreement with health care decisions. Be responsible for your own actions if you refuse treatment or do not follow the instructions of those providing care.
  • Ask for pain relief intervention when pain first begins and discuss pain management options to assist in development of a pain management plan.
  • Keep appointments for follow-up care or notify the clinic in advance if a cancellation is necessary.
  • Comply with all the hospital’s rules and regulations as they relate to your care, safety and conduct as a patient. Be considerate of hospital personnel and property as well as other patients; respect their privacy and property.
  • Provide the hospital with complete and accurate insurance and financial information and comply with agreed-upon payment arrangements.
  • Westlake Hospital has an obligation to inform all patients with regard to patient rights and an obligation to respect such rights. Patients also have responsibilities to other patients, Westlake staff, the physicians and other health care providers.
  • The right to communicate complaints or grievances regarding his or her care to his or her physician, nurse, nurse team member, consumer advocate or hospital management or administration. This may be done orally or in writing. A written complaint can be made to the Patient Relations Department at Westlake Hospital, 1225 W. Lake Street, Melrose Park, Illinois 60160.
